Eligibility
Men and women who are: - Justice involved. Newly released or formerly incarcerated (jail or prison) within the last 18 months. - Homeless or are at risk of homelessness/have unstable housing - Have a mental and/or substance use disorder - Interested in behavioral health and/or reentry support services - Medi-Cal eligible or current Medi-Cal members (Medi-Cal enrollment support can be provided if necessary)

Supports individuals as they transition from incarceration back into the community. Provides mentoring, resources, and comprehensive services for men and women of all faiths. Aims to be a bridge of support and compassion during the transition by linking individuals to housing, behavioral health services, benefits, etc.
