Eligibility
In order to be considered for services, clients must have Medi-Cal and active Central California Alliance for Health (CCAH) coverage.
Eligible populations include:
- Individuals and families experiencing homelessness or at risk of losing housing within 30 days
- Victims of domestic violence
- Adults with Serious Mental Illness (SMI) or Substance Use Disorder (SUD)
- Adult high utilizers of the health care system (multiple hospital admissions)
