H4HP Coordinated Entry
Coordinated Entry is a systemic approach to connecting people experiencing homelessness with available assistance in the community. It is an important way to ensure fair and equitable access to limited housing resources and is a Federal and State requirement. The goals of the new Coordinated Entry System are to:
(1) Facilitate connections to mainstream and community services for as many persons experiencing homelessness as local resources allow;
(2) Streamline the process for matching to limited housing resources within the Housing for Health Partnership network (CoC); and
(3) Prioritize resources for households with the most significant barriers to getting and keeping housing and for those with the greatest personal health and safety risks.
H4HP Connectors may work as part of an outreach team, drop-in center, multi-service center, or other program serving people experiencing or at risk of homelessness. They engage with persons experiencing homelessness, build rapport, enroll participants in HMIS programs and collect participant data.
Connectors offer problem solving assistance with all participants and may conduct a Housing Needs Assessment (HNA). The HNA provides Connectors and participants with information needed to create and act together on an individualized Housing Action Plan and provides information to determine which participants are eligible and prioritized for H4H-supported housing and services.
Only the highest priority households are added to the queue for the system’s limited housing resources.

Provides home delivered meals for adults with a disability and seniors. Offers seven meals and six delivery days per week.

Manages benefit thrift stores. Accepts donations of clothing, shoes, bedding, linens, furniture, appliances, computers, and household items. Resells items in support of Goodwill career services programs.

Developmental Disability Services
Section 8 Program
Administers Housing Choice Voucher and Project Based Voucher programs for income eligible households. Housing Choice Voucher program subsidizes part of the total rent. Housing Choice Voucher applicants are responsible for housing search and selection. Project Based Voucher program subsidizes a portion of rent at designated properties.
Civil Division
Provides legal processing services in the areas of family law. Helps with concerns related to the following:
- Probate guardianship/conservatorship
- Mental health
- General civil disputes and small claims
- Unlawful detainers
- Judicial consent for marriage of underage parties.
Also provides services for juvenile dependency and juvenile delinquency. The family court services provide referrals for same-day mediation. Includes child custody and visitation mediation.

Low Income Home Energy Assistance Program (LIHEAP)
Distributes financial assistance for home energy bills when funding is available. Provides utility bill payment assistance for gas and electric services. Offers heating fuel payment assistance for wood and propane.

Provides food to individuals and families in need. The pantry offers food staples (such as rice, beans, bread, and cereals) and fresh produce. Food distribution is subject to availability.
