Recuperative Care Center

The Recuperative Care Center is an innovative medical respite program located on the Coral Street campus. This program is a collaboration between Housing Matters and the County of Santa Cruz Homeless Persons’ Health Project, Dominican and Watsonville hospitals, Palo Alto Medical Foundation (PAMF), Hospice of Santa Cruz County, Kaiser Permanente, and Central California Alliance for Health. Up to 12 individuals experiencing homelessness can stay at a time and recover/stabilize while receiving integrated social services including housing planning, mental health care, benefits enrollment, and substance abuse treatment. The Recuperative Care Center aims to reduce recovery time from significant medical events and to decrease the likelihood of recurring hospital stays.

Transition Age Youth

Serves current and former foster youth age 15-24 in making a healthy transition to adulthood. Programs include counseling, skill-building, a drop-in center and subsidized supportive housing.

TAY programs include:

- The Independent Living Program (ILP) serves current and former foster and probation placement youth age 15-21. Assists youth to develop independent living skills to successfully transition to self-sufficiency and includes services such as one-on-one counseling, assistance in obtaining ID cards, driving permits and licenses and workshops on topics that assist youth in achieving personal, education and vocational goals.

- The Transitional Housing Program is a supportive housing program for former non-minor dependents and foster youth between the ages 18-24. Supports with financial assistance, educational expenses, emotional support, life skills coaching, and referral to community resources to help them achieve the financial stability.

Teenage Pregnancy and Parenting Program (TAPP)

Focuses on the reduction in second births or prevention of the first, enrollment or retention in schools, reduction of low birth weights, and reduction in risk behaviors of adolescents. Acts as a Teen and Family Resource Center that provides a myriad of support service programming ranging from child development and child care to nutrition and health education, job readiness, mental health, relationship violence prevention, and academic counseling.

Serves and empowers vulnerable communities in Solano County through education and outreach to raise awareness about disaster preparedness. Teaches five steps to emergency preparedness that include:
  • Get alerts to know what to do.
  • Make a plan to protect loved ones.
  • Pack a go bag with needed things.
  • Build a stay box for when individuals cannot leave.
  • Help friends and neighbors get ready.

CalFresh

Provides financial assistance for eligible residents to purchase healthy and nutritious foods. Offers three day expedited service for some special emergency situations. Issues Electronic Benefits Transfer (EBT) cards with a monthly credit to purchase groceries. EBT cards accepted at participating retailers including grocery stores and farmers markets. CalFresh is the state version of the Supplemental Nutrition Assistance Program (SNAP). EBT Online Purchases CalFresh EBT has extended service for online use at approved retailers. Recipients can purchase eligible groceries at participating stores for delivery and pickup. Restaurant Meals Program CalFresh EBT cards accepted at various locations participating in the Restaurant Meals Program. CalFresh eligible recipients are able to buy prepared meals at participating restaurant vendors. CalFresh recipients experiencing homelessness, with a disability, or age 60 and older eligible.
