Youth Homeless Response Team

Transitional Age Youth Housing Navigation Program

Provides housing navigation, individualized case management, and resource connections for young adults ages 18-24 who have experienced the foster care system. Offers life skills training to assist participants with obtaining and maintaining stable housing. Delivers mentorship and tailored transition planning to support long-term independence.
